Eligibility

To be eligible for services under the Commonwealth Home Support Program (CHSP) and the Home and Community Care (HACC) services you must:

  • Live in the community;
  • Socially Isolated
  • Have difficulty performing activities of daily living without help due to functional limitations;
  • Be at risk of admission to long term residential care without assistance from Commonwealth HACC services.

 Eligibility is not based solely on age, but on the level of difficulty in carrying out activities of daily living without help or supervision. Activities of daily living include personal care; dressing; preparing meals; house cleaning and maintenance; and using public transport.

All guests who access the Centre may live alone and have become socially isolated or they may live with family or a Carer.

  • Guests wishing to attend the Centre under CHSP/HACC programs must be assessed as CHSP/HACC eligible prior to attending
  • If you are in receipt of and  receiving other Government funded packages e.g. EACH this package will bear the costs of the attendance fee.
  • Access to the Day Respite Centre is on a non-discriminatory basis for all CHSP/HACC eligible guests:
    • Frail aged persons (over 65 years)
    • Younger persons with a disability including Qld Veterans (under 65 years)
    • Aboriginal and Torres Strait Islander people
  • Support is available for people from CALD groups and non-english speaking backgrounds, including those with special needs, people suffering Dementia and people who are financially disadvantaged.
  • Referrals may be made through My Aged Care, Health Care professionals, relatives or friends.  Self referrals are also welcome.
